Fit Note Certificates

Fit notes (sometimes called a 'sick note') can now be requested online.

The note will usually be sent to your mobile phone. Please allow 7 working days to receive this.

Please take the time to provide as much detail as possible to avoid delay i.e. the nature of the illness, the impact it is having on you and when you expect to return to work. If the information is unsatisfactory, you may be asked to arrange a formal GP appointment.

Please do not use this as a channel to list your other medical problems or queries. These forms will be used for the issue of sick notes only.

If you have been ill for 7 days or less you don’t need to see a Doctor. You can complete a Self-Certification form yourself. 

If your employer doesn’t have its own form you can download the Self Certification Form above. Please print it, fill it in and hand it in to your employer.

If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness to Work’ Certificates.

To request this please fill the form above. The doctor will need as much detail about your illness to be able to help complete the certificate so please take the time provide this.

If you have already had a Fit Note for this illness your Doctor may not need to see you to issue an additional Sick Note.
