Important Update: Appointment Booking Process (Week of December 2nd) 

PLEASE NOTE: This trial has now been closed and we are evaluating its success. Patchs will re-open on Monday morning and in the meantime please phone 111 if you have an urgent medical request, or 999 in an emergency.

 

 

Starting the week of December 2nd, we are trialling a new appointment booking system. This is part of our ongoing efforts to improve service efficiency and your experience with us. 

For the duration of this trial, all appointment requests will need to be submitted through our Patchs request form. You can use this form to request an appointment for yourself or for someone you care for. 

Our triage team will review your request and respond Monday to Friday, between 8am and 6.30pm, within 48 hours. 

If you need assistance with the online form or prefer to book in person, you can: 

  • Call us at 0121 796 2777 for support. 
  • Visit one of the GPS sites in person. 

This is a trial period, and we will notify you of any changes to our process after the test week. If your situation is urgent, please select the relevant options below.

Immediate action required: Call 999 or go to A&E now if:

  • you or someone you know needs immediate help 
  • you have seriously harmed yourself - for example, by taking a drug overdose 

A mental health emergency should be taken as seriously as a medical emergency.

Non-urgent advice: Use NHS 111 if:

  • You need help now, but it's not an emergency 

There will be someone to provide you with advice and to direct you to a clinician if it is necessary.

Appointments can be made in three ways:

  • Through the online booking facility - Patchs
  • On the telephone - 0121 796 2777
  • By visiting the surgery

When you get in touch, we’ll ask what you need help with. We will use the information you give us to choose the most suitable doctor, nurse or health professional to help you.

We do not accept appointment requests via email/letters.

Your Appointment

However you choose to contact us, your issue will be triaged and passed to the most appropriately trained personnel, we may offer you a consultation:

  • by phone
  • face to face at the surgery
  • on a video call
  • by text or email

Appointments by phone, video call or by text or email can be more flexible and often means you get help sooner.

The NHS App has many different features that can help the patients manage their helathcare without having to wait on the telephone line.

If you’re a patient at our practice, you can use the NHS App to access a range of NHS services on your smartphone or tablet.

It doesn’t replace existing services and you can still contact us in the usual ways.

But, once you have verified your identity in the app, you will have easy, 24/7 access to a growing range of health services and information. You can: 

  • order repeat prescriptions
  • access NHS 111 online
  • find NHS services
  • view your health records
  • book appointments
  • and much more… 

If you have any problems using the NHS App, you can select ‘help question mark icon’ in the top right-hand corner of the app or visit nhs-app-help-and-support.

 

Non-urgent advice: Sick Note Requests

7 days off sick or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. This is called self-certification.

More than 7 days off sick

If you’re off work sick for more than 7 days, your employer will usually ask for a sick note (or Statement of Fitness for Work) from a healthcare professional.

Extension of an existing sick note

If you have already had a sick note for your illness, a GP may not need to see you again to issue an additional sick note.

How can I get a sick note?

If you need a sick note, you can contact the surgery. The best way is to use Patchs – our online booking platform.

Non-urgent advice: Text Reminder Service

We have a texting service which allows you to receive confirmation and reminders about your appointments.

To have this service you will need to register by completing a consent form.

Please remember to update your contact details with us when you change address, telephone numbers and email address.

Non-urgent advice: Cancelling/changing an appointment

To cancel/change your appointment:

  • follow the instructions in your appointment reminder text message
  • use your NHS account (through the NHS website or NHS App)
  • use Patchs
  • phone us on 0121 796 2777

Non-urgent advice: Home visits

You may only request a home visit if you are housebound or are too ill to visit the practice. A visit will only be made if your medical condition requires it. Please be prepared to provide suitable details to help us with this process.

You can also be visited at home by a community nurse or a health visitor if you have recently had a baby.

To request a home visit, please:

  • Call the surgery on 0121 796 2777 before 11:00.
